We have the following care providers on our team:
Skilled Nurses — Licensed experts who adhere to the doctor’s plan of care. Services include:
- Consistent, thorough care
- Monitoring vital signs
- Medication oversight
- Symptom control
- Wound care
- Pain and symptom management
Certified Nursing Assistants/Hospice Aides – Certified professionals who work very closely with our nurses to ensure there are not any gaps in your loved one’s care. Services include:
- Assisting in personal care
- Light housekeeping
- Life enhancement activities such as reading to the patient, writing letters, and going for walks.
Medical Social Workers – Licensed experts who go beyond traditional health care so your loved one is connected to all the resources they need to be as comfortable as possible. Services include:
- Social and emotional support
- Assisting with community resource scheduling
Chaplain – Treating the whole person includes helping to meet the spiritual needs of the patient and family. A chaplain treats the whole person — mind, body, and spirit. While assessing the patient’s and family’s spiritual needs, they provide direct counseling, consult with community clergy, and provide bereavement support.
Medical Director – The Medical Director leads the hospice care team in developing a plan of care to meet each patient’s specific needs, provides consultation to physicians regarding hospice care, and certifies terminal prognosis as part of the hospice eligibility process.
